In British English, the "ER" is called either "the casualty department" (or simply "casualty") OR "Accident and Emergency" (A&E).

There is nothing in this email which suggests that Sophie is an elevator repairperson, or that she fixes lifts or elevators.
There is no reason to assume that Sophie is someone who cooks pizza for a living. If she were, then she probably wouldn't tell Bob that he could buy pizzas for their dinner!
This is the best choice here. Sophie talks about working in the Emergency Room, so we know that she must be working in a hospital, and is probably a "doctor" or a "nurse". Doctors and nurses are medical professionals who treat you when you are ill or have an accident.
There is nothing in this email which suggests that Sophie is a mechanic or technician.
