He is acknowledging that he was very slow to realize that she had personal problems.
2 middle-aged: A "middle-aged" person generally refers to someone between 45 and 65 years old - they are in the intermediate stage between being young and old. "Middle-aged" is not usually a derogatory term, but, knowing how sensitive people can be about their age, it should be used with discretion!
