1 flames: "Flames" are parts of a fire, they are red, orange and yellow and move about. In this dialogue, Dr. Loblaw requests a "flame" from the nurse for his operation on Kevin. As a tool, a flame can be used to burn or join metal together.
2 hammer: A "hammer" is a tool used for driving "nails" into various surfaces such as wood, metal or concrete. The head of a hammer is generally a flat metal surface on one side, with a design meant to remove nails on the reverse side. In this dialogue, Dr. Loblaw requests a hammer from the nurse for his operation on Kevin.
3 nail: "Nails" are thin, cylindrical pieces of metal with sharpened ends on one side and a flat head on the other. They are driven or beaten into surfaces such as wood or metal with "hammers", a tool used by Dr. Loblaw during this operation.

5 tools: "Tools" are instruments generally used for the repair or construction of objects such as furniture (among many other uses). A "tool" is also a general term for any type of instrument which serves a specific purpose, for example: the tools of a writer are pen and paper. Although this term wasn't used in the dialogue, it could replace Dr. Loblaw's term "instruments". Furthermore, all of Dr. Loblaw's requests in this scene are "tools": a hammer, a scalpel, a flame, etc.
